- Two copies of the duly-accomplished Postal ID application form.
- Improved or Basic Postal ID Card, whichever is applicable.
Requirements for Renewal/Replacement With a Change in Biographic Data
- Two copies of the duly-accomplished Postal ID application form.
- Improved or Basic Postal ID Card, whichever is applicable.
- Additional documents to support the change in the biographic data.
