Important Reminders
- You can download the form and accomplish it online/electronically. Download the latest version of the application form here.
- You can also accomplish the form by hand.
- The application form must be printed on A4-sized paper.
- You can accomplish the form using either the English or Korean language.
- Write in all capital letters when filling out the form.
- The Embassy doesn’t have rules regarding font style and size. However, this is a formal transaction so choose font size of at least 12 and font styles commonly used in business letters like Arial, Calibri, Tahoma, Times New Roman, etc. If you’re having difficulty adjusting the font settings, ask assistance from the travel agency so they can provide you with a ready-to-answer application form in PDF format.
- The answers should be TYPEWRITTEN/COMPUTERIZED as most travel agencies no longer accept forms that are handwritten.
- Write N/A if an item does not apply to you.
- A checkmark (√) should be used when filling out items with multiple selections.
- The accomplished form should have no erasures.
How To Fill Out the South Korean Visa Application Form: A Step-by-Step Guide
We’ll teach you how to accomplish the application form by focusing on one section at a time. Take note that RED text in the following screenshots is for emphasis only.
1. Personal Details
1.1 Input your Last Name and then your First Name.
1.2 Indicate your Chinese name if you have one. Write N/A if you have none.
1.3 Put a check on the appropriate space if you’re a Male or a Female.
1.4 Write your birthday in this format: Year/Month/Day.
1.5 Indicate your nationality.
1.6 Write the name of the country where you were born.
1.7 Write your National Identity Number if you have one. Write N/A if you have none.
1.8 Check ‘Yes’ if you have used other names to enter or depart Korea. Indicate your previous Family Name and Given Name. Check ‘No’ if you haven’t used other names and write N/A on the Family Name and Given Name.
1.9 Check ‘Yes’ if you’re a citizen of another country and indicate which country. Check ‘No’ if you’re not and write N/A on the space for the name of the country.
Guidelines for the Photo:
- The picture must be taken within 6 months from the date of application.
- The photo should use a white background.
- Don’t wear a hat.
- Your face should be neutral in the photo.
- Paste the photo on the application form.

2. Passport Information
2.1 Put a check on the type of passport that you currently have.
2.2 Write your Passport Number.
2.3 Indicate the country where the passport was issued.
2.4 Indicate the specific location or office where your passport was issued.
2.5 Enter the date your passport was issued.
2.6 Enter your passport’s expiry date. Make sure that your passport has at least 6 months of validity left.
2.7 Check ‘No’ if you have no other valid passports and leave the ‘Details’ section blank. Check ‘Yes’ if you have other valid passports and indicate the Passport Type, Passport Number, Country, and Expiry Date.
3. Contact Information
3.1 Write your permanent address in your home country.
3.2 You should only fill this out if it’s different from your permanent address.
3.3 Write your cellphone number.
3.4 Write your telephone number.
3.5 Indicate your active e-mail address.
3.6 Provide your emergency contact information with the Full Name, Country of Residence, Telephone Number, and your relationship with the person.
4. Marital Status and Family Details
4.1 Put a check on your marital status.
4.2 Leave this section blank if you have checked ‘Divorced’ or ‘Single.’ If you checked ‘Married,’ provide the following details of your spouse: Family Name, Given Name, Date of Birth, Nationality, Residential Address, and Contact Number.
4.3 Check ‘No’ if you don’t have any children yet. Check ‘Yes’ if you already have children and indicate the number of children that you have.
5. Education
5.1 Check the highest level of education that you have completed. If you’ve checked ‘Other,’ provide specific details.
5.2 Write the name of your school.
5.3 Write your school’s location.
6. Employment
6.1 Check the type of employment you’re currently under.
6.2 Provide your employment details including the following: Name of Company/Institute/School, Your Position, Address of Company/Institute/School, and Telephone Number.
7. Details of Visit
7.1 From the list of options, check the purpose of your visit to South Korea.
7.2 Indicate the length of your stay for your trip to South Korea.
7.3 Indicate the intended date of entry. Make sure that it’s within three months from the date of application.
7.4 Write your intended address in South Korea during your stay. You don’t need to have prior reservations or bookings.
7.5 Indicate the contact number that you’ll be using during your trip.
7.6 Check ‘Yes’ if you have previously traveled to South Korea in the last 5 years and provide additional details like the number of times you’ve visited and the purpose of your most recent visit. Check ‘No’ if it’s your first trip to South Korea.
7.7 Check ‘Yes’ if you have previously traveled outside your country of residence and provide additional details like the name of the country you have visited, the purpose of your visit, and the period of your stay. Otherwise, check ‘No.’
7.8 Check ‘Yes’ if you’re traveling with any of your family members (spouse, parent, children, siblings). You also have to provide the following information of your companion/s: Full Name, Date of Birth, Nationality, and relationship to you. Check ‘No’ if you’re not traveling with a family member and write N/A on the ‘Details’ section.
8. Details of Sponsor
8.1 Check ‘Yes’ if you have a sponsor in South Korea and provide the following details of your sponsor: Name, Date of Birth or Business Registration Number, Relationship to you, Address, and Telephone Number. Otherwise, check ‘No.’
9. Funding Details
9.1 Indicate the estimated cost of your entire trip to South Korea. For reference, the average budget per day is around Php 5,000. Make sure that you’ll be able to show that you have enough funds in your bank account for the whole duration of your trip.
9.2 Mention who will fund your trip (including yourself or an institute) and provide the following details: Name of Person/Company/Institute, Relationship to you, Type of Support, and Contact Number.
10. Assistance with this Form
10.1 Check ‘Yes’ if someone assisted you in filling out this form and provide the following details: Full Name, Date of Birth, Telephone Number, and Relationship to you. Otherwise, check ‘No.’
11. Declaration
Declare that all the details and information you have provided in the application form are true and correct based on your knowledge and belief.
Indicate the date you will submit your VISA application and provide your signature. If you’re under 17 years old, your parent or legal guardian will sign the form for you.
